If you visit our login page, we will set a temporary cookie to determine if your browser accepts cookies. This cookie contains no personal data and is discarded when you close your browser.
When you log in, we will also set up several cookies to save your login information and your screen display choices. Login cookies last for two days, and screen options cookies last for a year. If you select “Remember Me”, your login will persist for two weeks. If you log out of your account, the login cookies will be removed.
If you edit or publish an article, an additional cookie will be saved in your browser. This cookie includes no personal data and simply indicates the post ID of the article you just edited. It expires after 1 day.
Controlled Substance Policy
- If your medication is lost, you should file a police report and notify both MindSight Health and Wellness and the pharmacy. Controlled medications WILL NOT be refilled earlier than their fill date. To minimize the risk of medication loss, store them in a secure place, out of children’s reach, and avoid carrying more medication than necessary during travel.
- For travel, plan accordingly – controlled substances will not be filled earlier than their refill date.
- Benzodiazepines: we may provide assistance for a short-term 1 – day prescriptions for special procedures such as MRI or flights. We do not initiate or maintain benzodiazepine treatment.
Cancellation Policy
- We require that you provide 48-hour notice should you need to cancel or reschedule an appointment. We understand that situations out of our control arise, but excessive (up to 3) episodes of late cancellations or missed appointments (no-show) may lead to termination of care. In the event of no response from the patient or guardian, a termination letter will be sent via certified mail.
If you are more than 10 minutes late to an appointment, your appointment will need to be rescheduled. It is imperative you arrive on time and consistently attend your appointments to help maximize your mental health outcomes.
A fee of $75 for evaluations and $50 for follow-up appointments will be applied for cancellations made with less than 48-hour notice from the appointment time.

Credit Card on-file Policy
- With offering telemedicine, it is mandatory for all patients to furnish a valid credit card when registering to access any and all of our services. We retains the right to utilize the credit card details provided for settling any outstanding balances related to current patient accounts. In the event that you, as the patient, fail to attend a scheduled appointment (“No-Show”) without providing a 48-hour notice for cancellation or rescheduling, MindSight Health and Wellness maintains the right to charge the credit card on file with a no-show and/or cancellation fee. A receipt documenting the transaction will be securely stored in your payment history.

Communication Policy
- Once your care is established, please use the practice’s portal for communication. Communication occurs within regular business hours unless alternative arrangements have been made. In circumstances requiring urgency to speak to your provider the practice number may be utilized otherwise the patient portal is the best means of communication.
*Please note that we do not offer urgent or emergent services. If you are experiencing a crisis or require immediate assistance, please dial 911 or visit the nearest emergency room. If you would like to speak with someone but are not in immediate danger, you can contact the National Suicide Prevention Lifeline at 988.
- Medication Refills Policy: Obtaining refills without a scheduled appointment does not align with best practices in treatment. As a standard, refills are addressed exclusively during appointments. Ensuring you have an adequate supply is your responsibility to prevent any interruption in care. Should you miss an appointment, it’s your responsibility to reschedule before your medication runs out.
In situations where an appointment is canceled or missed, and a refill is required, an appointment will be required to be scheduled and a brief bridge prescription provided until the next appointment. We encourage planning ahead to ensure appointments align with refill needs. A bridge prescription will only be provided one time.
Please note that obtaining medications may be delayed by prior authorizations, and we have no control over the duration of insurance authorization processes.
- Confidentiality and Limits to Confidentiality
- Any information disclosed to MindSight Health and Wellness and all services provided remains strictly confidential. If you wish to obtain your records or have them sent to another provider or entity, please utilize the release of information form. Your information is not shared with any other party without your explicit consent, except when mandated by law or necessary for billing purposes.
- Any release of information may be revoked at any time by submitting written request through your patient portal.
MindSight Health and Wellness is required to reveal information under the following circumstances:
- When a patient poses a significant threat to themselves or others
- In cases of abuse or neglect involving children, elders, or intellectually disabled individuals
- Upon receiving a court order
- When actions are deemed necessary for collecting payment
- If an insurance company requests information for reimbursement of services
Emergency Contact Release of Information: We require an emergency contact be provided and with this requires an active signed release of information form.
Telemedicine Policy

In a telehealth or telemedicine session, you’ll connect with your provider securely through a HIPAA-compliant video link on your computer, phone, or tablet. The link will be included in the email reminder sent to you prior to your appointment. We encourage you to click on the link and test it before our scheduled appointment to ensure a smooth experience.
LOCATION: While these appointments are conducted virtually, medical licensing regulations prohibit practitioners from meeting with patients in states where they are not licensed. Therefore, please ensure you are located in Tennessee for your session.
Environment: Please ensure you are in a private space that allows you to speak open and honestly about your treatment. MHW reserves the right to ask you to span the environment around you to ensure it is safe and protects your confidentiality. The environment should be free from interruptions and with a good internet connection. If your environment or location is deemed to not be private and would not protect your confidentiality your appointment will be rescheduled. Telemedicine appointments will not be conducted while driving.
